Study
Study title: Online handwritten-signature verification
Researcher/contact: ____________________________________
Institution/contact: ___________________________________
What participation involves
You may be asked to provide several genuine signatures and, if applicable, clearly identified simulated forgeries. The capture may include pen/cursor position, timestamps, pressure, and contact area when the device provides them. The purpose is to evaluate a research algorithm; the system is experimental and must not be used for legal identity decisions.
Data use and protection
The study team will use the collected data for the stated research purpose. Signature dynamics can be sensitive biometric information. The project should use participant codes instead of names, limit access, encrypt stored templates, set a deletion date, and explain any sharing or publication of derived results. The researcher must fill in the actual retention and deletion arrangements before presenting this form.
Voluntary participation
Participation is voluntary. You may stop at any time and may request removal of your identifiable raw records until: ____________________. Explain any limits on withdrawal after data have been anonymized or published.
Consent
I have read and understood the information above, had the opportunity to ask questions, and voluntarily agree to participate.
